Please email your appointment request including the type of service you are looking for including your name and phone number, store location and we will confirm your request.

Please provide us with the details of the services (Custom Orthotics- Compression Stockings- Shoe Fitting- Assessment) you are interested in and the time you would like to come in.  

If you are trying to claim through an extended health benefits provider please provide the name of the Insurer.
